OPTIMA reinforces the clinical value of Prosigna and the ≤60 threshold to identify patients unlikely to derive meaningful additional benefit from chemotherapy. Although encouraging, the premenopausal sample size in OPTIMA alone is not sufficient to provide a definitive answer: definitive confidence for premenopausal patients will require OPTIMA + premenopausal extension + OPTIMA-YOUNG data.
